Screening for G.D.M
 
Indications for Screening for G.D.M
 
  • Age above 25 yrs.
  • Family history of diabetes
  • Obesity (B. M. I. – above 27 kg/m2)
  • Glycosuria on two or more occasion during current pregnancy
  • Polyhydroamnios in current pregnancy
  • Large for gestational age baby in current pregnancy
  • Past H/O macrosomia (>4kg.)
  • Past H/O still birth/neonatal death
  • Past H/O gestational diabetes / glucose intolerance
  • Ethnic / Racial group with a high prevalence of diabetes
Procedure
 
  • Performed at 24 – 28 weeks
  • Any time of day, irrespective of food
  • Ingest 50 grams of glucose and do blood glucose after one hour
  • Value above 140 mg% indicates the need for a 100 gm 3 hours OGTT (Oral Glucose Tolerance Test)
Diagnostic Criteria For G. D. M.
 
Three hours O. G. T. T. Procedure
 
  • Over night fasting of 8 – 10 hours
  • Do fasting blood glucose
  • Ingest 100 grams of Glucose with water
  • Perform blood glucose every hour for 3 hours
  • Patient should sit quietly during the test
  • As per O’sullivan & Mahan (Modified by Carpenter & Coustan) if any two or more of the four results exceed the cut-off values the test is positive for G. D. M.
  • If one abnormal value is seen, it is recommended that the test be repeated one month later.
